FHI 360 is seeking a qualified candidate for the position of Active Case Management Coordinator.
The Case Management Coordinator under the supervision of the HIV Advisor will be responsible for capacity building of implementing partners on active case management with the objective of reducing interruptions in antiretroviral therapy among PLHIV, improving the quality of psychosocial support and counseling. He/she will work closely with FHI 360 technical officers and CSO partners to provide feedback on whether clients are receiving the standard quality of care and participate in quality improvement activities.
ESSENTIAL JOB FUNCTIONS, DUTIES AND RESPONSIBILITIES
Provide coordination support to HIV testing sites and care and treatment sites to facilitate linkage of all newly diagnosed PLHIV into HIV care and treatment.
Train and mentor health facilities to profile and provide targeted support to clients at risk of interrupting HIV treatment.
Train and mentor health facilities, CSO partners to improve adherence counseling and support to PLHIV.
Build the capacity of CSO partners and health facilities on psychosocial support, trauma counseling for survivors of gender-based violence, and preventing burnout among caregivers.
Coordinate with the monitoring and evaluation team to maintain a database of registered PLHIV who have interrupted treatment or are declared lost to follow-up (LTFU) by care and treatment sites to inform active follow-up activities.
Mentor and actively follow up with care and treatment sites to implement efficient tracking systems for clients missing from treatment.
Coordinate inter-facility and community-level efforts for tracking clients missing from treatment.
Conduct bi-weekly calls and visits to all facilities to make sure they submit tracking plans for funding ACM activities; monitor and support sites to ensure uninterrupted ACM activities.
Provide technical support on data recording and reporting to ensure that feedback from tracking activities is appropriately and correctly recorded in clinic logbooks and HIV program database.
Review HIV case reporting forms each week for completeness and aggregate all data on new cases requiring to follow up with a view to accounting for all newly identified PLHIV.
Facilitate site-level monthly case management meetings and regular provincial-level meetings for quality improvement, share experiences and provide feedback to managers.
Document and share regular reports on the progress, lessons learned, and challenges related to active case management and make appropriate recommendations to NCD PHA and FHI 360
KNOWLEDGE, SKILLS, ABILITIES AND COMPLEXITY
Ability to coordinate activities with multiple clinics and to provide mentoring support.
Good interpersonal skills.
Technical knowledge of mental well-being, psychosocial support, and counseling.
Basic M&E principles and data management skills.
Well-developed written and oral communication skills.
Proficiency in the use of computer software, including Microsoft Office applications.
MINIMUM REQUIREMENTS
Diploma or Bachelorโs degree in health or social sciences.
At least 5 years of experience counseling and providing psychosocial support to vulnerable populations.
Familiarity with the PNG national HIV program, Continuum of prevention, care and treatment package and the health delivery system in PNG.
Good networking relations with health facility and community level stakeholders in service delivery.
LOCATION OF EMPLOYMENT:
The employee will be based at FHI 360 country office in the National Capital District, Papua New Guinea.
