PIH currently has implementation programs in Haiti, Rwanda, Lesotho, Liberia, Malawi, Peru, Mexico, Russia, Sierra Leone and Navajo Nation. Through our partnership with Harvard Medical School and the Brigham and Womenโ€™s Hospital, we support a wide range of programs in dozens of other countries around the world.

Our Work In Sierra Leone:

Partners In Health (PIH) began supporting the government of Sierra Leone in response to the Ebola epidemic in October 2014. Recognizing the large need for continued investments in the health system after Ebola, PIH is committed to long-term health system strengthening in support of the Ministry of Health and Sanitation (MOHS), guided by the same principles as our emergency response and our work around the world: public sector accompaniment, integrated and comprehensive services, social justice, nimbleness and flexibility, partnership, community ownership, and evidence-based interventions. In the aftermath of the epidemic, PIH is committed to supporting the delivery of comprehensive health services in Sierra Leone, from the community level to the health center and to referral hospitals.

Position Overview

The Acute Needs Program Surveyor assess client household needs using the ANP Assessment. The assessment reviews socioeconomic factors, access to a caregiver, distances to healthcare services, assesses if the lodging is in need of repairs, etc. This is a Community Based Position. He /She works closely with clinicians, CHWs, Field supervisors, Facility based officers and social workers to ensure timely assessment of clients in the communities or facilities prior to discharge.

Essential Duties and Responsibilities:

  • Weekly collection of ANP referral forms from all PIH supported facilities for home assessment
  • Keep regular and updated records of those that receive the ANP home survey
  • Weekly presentation to the ANP committee of findings for each client assessed
  • Receive patient names, address and other necessary information of people to be surveyed from the ANP Program Officer
  • Learn how to use assessment tool with objectivity, give feedback on the assessment tool and assessment process to the program Officer
  • Develop a weekly schedule with the ANP officer for completing assessment
  • Advocate on behalf of patients to the ANP Officer and ANP committee
  • Maintain confidentiality and be empathetic towards patients and families
  • Adhere to PIH policy and do not accept gifts or payments from clients in the community
  • Work closely with CHWs and field supervisors to assess homes of HIV /TB patients for community support
  • Weekly phone calls to clients to seek consent for approval of home assessment
  • Attend all meetings and trainings organized by the Community Based Program
  • In โ€“ patients assessment at KGH and WBC as requested by clinicians
  • Complete the CommCare mobile phone based assessment and then upload it to the CommCareHQ website where it will be accessed by the Acute Needs Program Officer

    Education and Work Experience Requirements

    • Preferably must have completed WASSCE or high school certificate.
      • Commitment to social justice and human right.
      • Should be an exemplary, honest, trustworthy, and respectful.
      • Must be able to read and write in English as well as add and subtract using basic maths.
      • Should be willing, able, and motivated to serve his/her community and dedicated to caring and supporting vulnerable patients, family members and the community at large.
      • Must be a permanent resident of the community and willing to work in it.
      • Should be able to perform specified tasks as outlined in the scope of work
      • Should be interested in community health and development
      • Should be a good mobilizer and communicator
      • Should be involved in community or survey projects in the past
      • Must be at least 18 yearsโ€™ old
      • Must be residing in Koidu town , Kono district
