Action Against Hunger-USA has over $100 million in programs, and approximately 1,800 permanent staff based in New York City, Washington D.C, Nairobi, and country offices. Summary of Position

A pharmacy technician will provide pharmaceutical care by dispensing medications, counseling patients, monitoring drug therapies, and collaborating with healthcare teams to optimize patient outcomes. He/She will play a crucial role in ensuring medication safety, promoting rational drug use, and providing evidence-based pharmaceutical services.

Purpose

The purpose of the pharmacist role is to deliver comprehensive pharmaceutical care to patients. He/She will be responsible for accurately dispensing medications, counseling patients on proper medication use and potential side effects, monitoring drug therapies, verifying prescriptions, and collaborating with healthcare professionals to ensure optimal medication management. Pharmacists contribute to patient safety, medication adherence, and the overall quality of healthcare services.

Engagement

The Pharmacists will engage in various activities to fulfill their role in pharmaceutical care. He/She will interact with patients, collaborate with healthcare teams, conduct medication reviews, provide drug information, oversee medication distribution, and contribute to medication safety protocols and quality improvement initiatives.

Essential Job Functions

Medication Dispensing and Verification: Accurately dispense medications, review prescriptions for accuracy and appropriateness, and ensure proper labeling and packaging. Patient Counseling: Provide patient-centered counseling on medication use, dosage instructions, potential side effects, and drug interactions. Address patient questions and concerns. Medication Therapy Management: Review medication profiles, conduct medication reviews, monitor drug therapies, and make recommendations to optimize patient outcomes, including dosage adjustments or therapeutic substitutions. Drug Information and Education: Provide accurate and timely drug information to healthcare professionals, patients, and caregivers. Stay updated with medication guidelines, clinical trials, and evidence-based practice. Collaborative Healthcare: Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated and comprehensive patient care. Participate in healthcare team meetings and contribute to treatment plans. Medication Safety and Quality Assurance: Implement medication safety protocols, perform medication error prevention strategies, and participate in quality improvement initiatives. Adhere to regulatory requirements and best practices. Prescription Review and Verification: Review and verify prescriptions for accuracy, appropriateness, and potential drug interactions. Consult with prescribers as needed. Recordkeeping and Documentation: Maintain accurate and complete patient records, including medication profiles, interventions, and communication with healthcare professionals. Adhere to privacy and confidentiality regulations.

Requirements

Required Qualifications and Professional Experience:

Bachelor degree of Pharmacy (PharmD) degree or equivalent qualification. Minimum of 5 years of experience working as a pharmacist in a clinical or community pharmacy setting.

Required Skills, Competencies & Attributes:

In-depth knowledge of medications, pharmacology, therapeutics, and pharmaceutical care principles. Strong understanding of drug interactions, adverse effects, and medication management strategies. Proficiency in using pharmacy software systems and technology. Excellent communication and interpersonal skills for patient counseling and collaboration with healthcare professionals. Attention to detail and accuracy in prescription review and medication dispensing. Analytical and critical thinking skills for medication therapy management and problem-solving. Strong organizational and time management skills to handle multiple responsibilities. Ethical conduct, professionalism, and commitment to patient safety and confidentiality. Continuous learning mindset to stay updated with advancements in pharmacy practice and medication therapies. Ability to work independently and as part of a multidisciplinary healthcare team. Flexibility and adaptability to changing healthcare environments and emerging healthcare trends.
